Periorbital hyperpigmentation is the medical term for dark circles under and around the eyes. A 2016 review in PMC (4756872) described it as having several causes, including pigmentation, vascular shadowing, volume loss, and skin laxity. Treatment depends on which cause dominates.

Prevalence runs as high as 30% in darker skin types, with higher rates reported in Indian populations. Genetic predisposition plays a major role in many cases.

Dark circles usually have more than one cause. A patient with genetic pigmentation may also have mild volume loss and thin skin that shows vessels beneath. Tackling only one cause often leaves a patient unhappy with the result.

A 2025 systematic review in Dermatologic Therapy (Wiley) found that multimodality approaches produce the best outcomes for periorbital hyperpigmentation.

Genetic dark circles often start in childhood or teenage years. Age-related pigmentation shows up later. Post-inflammatory pigment follows eczema, allergies, or other inflammation. Hormonal pigment (melasma) has its own pattern.

Identifying What Is Causing Yours

True pigmentation darkens the skin itself, regardless of lighting or angle. Stretching the skin gently does not change the color. Dermatologists can use a Wood's lamp or a small dermoscope to confirm depth.

Pigment that lives in the upper layer of the skin (epidermis) responds to topicals faster than pigment that sits deeper (dermis).

Vascular dark circles have a bluish or purple tint that changes with lighting. Thin skin allows underlying veins to show through. Tiredness, crying, and allergies can worsen the look temporarily.

Stretching the lower lid skin in good lighting often diminishes vascular dark circles without a matching drop in true pigmentation.

The tear trough hollow creates shadow that reads as a dark circle. Even perfectly clear skin can look dark because of the contour, especially with overhead lighting. A small amount of well-placed filler can erase what looks like pigment but is actually shadow.

Addressing the Drug-Induced Type

Topical prostaglandin analogs, such as bimatoprost eye drops used for glaucoma or for lash enhancement, can cause periorbital hyperpigmentation within 3 to 8 weeks of use (AAO EyeNet, 2013). The pigmentation may be reversible after you stop the medication.

If you use these products, check whether new dark circles began with the treatment. Your prescriber can discuss alternatives if the cosmetic impact is significant.

Some topical steroids, hormonal medications, and chemotherapy drugs can cause or worsen pigmentation. Share your full medication list with your dermatologist so they can identify possible contributors.

Photo-sensitizing medications (certain antibiotics, diuretics) increase UV-driven pigment and indirectly worsen dark circles.

Allergic shiners (darkened lower lids from chronic nasal congestion) respond to treating the underlying allergy. Patients who rub their eyes from dry eye, allergies, or contact lens issues develop post-inflammatory pigment in the thin skin. Stopping the trigger is often the most effective treatment.

Topical Treatments

Hydroquinone 4% is the benchmark topical for melanin-driven dark circles. Around the eye, providers usually prescribe lower concentrations (2%) for cycled use (8 to 12 weeks on, then a break). Kojic acid, azelaic acid, alpha arbutin, and tranexamic acid are additional topical options that work through slightly different pathways.

Azelaic acid is especially useful for post-inflammatory cases.

Topical retinoids speed cell turnover and help fade pigment over time. Tretinoin 0.02% is a common starting strength for the delicate eye area. Vitamin C is an antioxidant that supports collagen, inhibits tyrosinase, and brightens over several months of use.

Introduce new actives one at a time. Layering too many at once often causes irritation that looks like worsening dark circles.

A typical morning routine: gentle cleanser, vitamin C serum, moisturizer, SPF. Evening: cleanser, retinoid (a few nights per week to start), moisturizer, and bleaching agent cycled per your provider's instructions. Pretreatment with tretinoin and hydroquinone for two to four weeks before chemical peels is recommended (Journal of Drugs in Dermatology, 2014).

In-Office Procedures

Lactic acid 15% plus TCA 3.75% is a combination shown to help periorbital hyperpigmentation. Glycolic acid 20% is another choice for mild cases. Peels are applied carefully to the thin eye-area skin, and multiple sessions spaced two to four weeks apart give the best outcome.

Downtime is usually a day or two of redness and mild flaking. Strict sun protection during and after a peel course is mandatory.

Q-switched ruby, Nd:YAG, and picosecond lasers target melanin directly. The periorbital area needs corneal shields and experienced operators. Lasers that target vessels (such as pulsed dye) can help vascular dark circles. IPL works for both pigment and vessels in some patients but is used cautiously around the eye.

Sublative fractional radiofrequency has also been studied for periorbital hyperpigmentation (PubMed 37753683, 2023) and may be a safer option for medium to dark skin tones.

Tear trough filler addresses shadow from hollowing. It does not treat pigment directly, but it often transforms how dark circles look in photos. PRP and mesotherapy with tranexamic acid plus vitamin C were studied in 2024 (PMC 11626328) and both reduced pigment in a split-site trial.

Surgical and Structural Options

Lower blepharoplasty can fix dark circles caused by fat bags that cast shadow, skin laxity that catches light unevenly, or festoons that trap dark-looking fluid. Surgery does not remove pigment directly, but it can transform the appearance when structure is the main cause.

Not every patient is a candidate. A careful exam tells whether structure or pigment is the bigger contributor.

For patients with severe volume loss and chronic hollow-driven dark circles, fat grafting is a long-lasting option. Autologous fat is harvested from another body area and layered in small amounts to rebuild support.

Fat grafting is less predictable than filler in retention, but it is integrated with your own tissue and tends to look natural.

A surgical correction of structure plus ongoing topical and in-office treatments for pigment gives the most complete result for multi-factor dark circles. Your oculoplastic surgeon plans the sequence based on healing and maintenance.

Preventing Worsening

Daily broad-spectrum SPF 30 or higher is the first-line preventive measure. UV exposure increases melanin and accelerates collagen breakdown. Sunglasses add a mechanical barrier and reduce squinting.

Mineral sunscreens with iron oxide provide visible light protection, which matters for pigment-prone skin.

Chronic sleep deprivation and stress worsen both vascular dark circles (through blood flow changes) and pigmentation (through cortisol and inflammation). Consistent sleep and stress reduction protect your investment in treatment.

Elevating your head slightly during sleep can reduce morning fluid pooling under the eyes.

Harsh eye makeup removal, aggressive under-eye rubbing, and heavy gravitational tugging on the lids can all worsen dark circles. Use gentle, oil-based removers and a light touch.

Patient Questions About Dark Circles

Overnight fluid can pool in the lower lids and make dark circles appear worse on waking. Elevating your pillow slightly, reducing salt at dinner, and cool compresses in the morning usually help. If the difference is dramatic and persists, ask your provider about festoons or allergies.

Caffeine can temporarily constrict vessels and reduce puffiness, which may help mild vascular dark circles. It does nothing for true pigmentation or structural hollows. Use it as one layer in a broader routine if you like, but expect modest results.

Genetic dark circles in kids are usually harmless but worth mentioning to a pediatrician. Allergic shiners from nasal congestion are common and improve with allergy management. New dark circles with other symptoms deserve a medical visit.

Concealer does not change pigment, but heavy daily rubbing to apply and remove it can worsen skin quality. Use a gentle tapping motion and a thin layer, then remove gently at night. Tinted sunscreen often provides light coverage with skin benefits.

If stretching your skin upward or tilting your head back lightens the circles, shadow from volume loss is a major factor and filler can help. If the color stays the same in both positions, filler alone will not change much. A careful consultation confirms the dominant cause.

Iron deficiency, dehydration, and very low antioxidant intake can worsen dark circles in some patients. Addressing nutritional gaps helps overall skin health. Diet alone rarely clears dark circles, but it is a useful part of the whole picture.

Cosmetic tattooing to mask dark circles is a risky choice. The thin skin can react unpredictably, color-matching is hard, and removal is difficult. Most specialists advise against it. Better tools exist.
